First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation when it is for a single extraction or uncomplicated pathology procedure. However, a notable medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. A preoperative consultation and physical examination is mandatory for patients undergoing IV sedation or general anesthesia for surgery.

Please assist us by providing the following information at the time of your consultation:

• Your surgical referral slip and any x-rays if applicable;
• A list of medications you are presently taking;
• If we are a participating provider with your medical insurance network or dental benefit plan, please bring your identification cards. Some procedures require being submitted to the medical plan while sometimes a dental benefit plan will request an EOB (explanation of benefits) from your medical carrier. Please be sure to bring BOTH cards to your consultation appointment;
• Photo Identification.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

To help keep your costs down, please bring x-rays if they have been taken by your dentist. We will do our best to use those x-rays, but sometimes, the x-rays taken elsewhere may not show us exactly what we need to see and we will need to take x-rays at our office.
